Sage Creek provides services for individuals with Developmental Disabilities that include Autism, Down Syndrome, Cerebral Palsy and Epilepsy. 

These services may include personal assistant care, transportation and shared living. In addition, we provide day habilitation in conjunction with our sister agency Twin Oaks Day Center.
